What Causes Visible Veins?

A telangiectasia, also called a spider vein, is a small dilated vein near the surface of the skin. A spider vein can develop anywhere on the body and become visible and unsightly. Often times, it’s not one, but many visible veins that cause problems for the skin’s appearance. Causes of visible veins may be due to acquired or congenital reasons. One example of a congenital cause is a port-wine stain. Examples of acquired causes include acne rosacea, topical use of corticosteroids, damage caused by sun or cold exposure, skin trauma, and venous reflux disease. Regardless of the causation, effective cosmetic vein treatments are available at HOGUE CLINICS using laser/light modalities, chemical ablation, thermal ablation, and surgical removal. Safe, effective cosmetic vein treatments is the priority.

Good Candidates

Good candidates for cosmetic vein treatment include healthy individuals with concerns involving the skin appearance of their legs, face, & breasts, even if they also have coexisting medical vein concerns. Usually, a medical vein condition requires treatment first, since it is often responsible for giving rise to the cosmetic, more peripheral concern. In a coexisting cosmetic and medical vein condition, the success of cosmetic vein treatment is often dependent on resolution or stabilization of the medical vein condition. The only way to determine whether one “is” or “is not” a good candidate for cosmetic vein treatment at HOGUE CLINICS is to have an onsite evaluation by one of our vein specialists.

  • While the ages of our patients vary, the majority of our patients are adults between the ages of 18 and 80 years. Older or younger individuals are welcome to come in for a FREE Vein Screening evaluation to determine whether or not they are a good candidate for vein treatment.
  • Female patients should not have a vein treatment when they are pregnant or breastfeeding.
  • Patients need to understand that vein treatments give limited results and only treat existing veins and will not stop the development or progression of future vein problems. For this reason, patients must keep in mind that multiple treatment sessions may be needed and may require periodic or additional vein treatments in the future.

Treatment Options


Unwanted surface veins on the legs and breasts can be eliminated with injection sclerotherapy using Asclera® (also known as polidocanol). Polidocanol was developed as a local anesthetic agent, so its use in sclerotherapy has an added benefit of numbing the pain associated with an injection. It is a prescription injectable medication that can only be provided by a healthcare provider. To treat spider veins, reticular veins, tributary veins, and varicose veins, polidocanol is injected into the targeted unwanted surface vein. It works by damaging the endothelium, the cells lining the inside of blood vessels. As a result, the targeted surface vein will eventually shrink, collapse, and disappear. Cosmetic injection sclerotherapy typically takes less than 60 minutes to perform, depending on the number of veins being treated. Once treated, a patient may return home with no downtime. Following treatment, patients are instructed to wear a compression stocking over the treated area for several days.


safe, effective, non-invasive method exists for treating unwanted surface veins on the legs, face, & breasts. This method, called transdermal laser therapy, uses laser energy emitted from a laser system to heat-shrink the targeted veins. Laser safety glasses must be worn by the patient during the transdermal laser therapy session. Since the laser energy must cross the outside skin layers from the epidermis to the dermis to the location where the surface veins reside, it is reserved for light-skinned individuals without recent sun exposure in the prior few weeks, since the pigment melanin is a competing chromophore or target for absorbing delivered laser energy.


Endovenous laser ablation (EVLA)Endovenous laser ablation (EVLA) is a vein treatment that uses laser energy delivered through a tiny optical fiber placed inside the vein lumen to thermally ablate a targeted vein. This 30-minute minimally-invasive, outpatient procedure with essentially no downtime is primarily reserved for treatment of varicose veins. When a patient with varicose veins has no medical signs or symptoms to support a medical condition, or if the patient’s insurer does not cover a specific varicose vein, then treatment of the vein is considered cosmetic or elective. Endovenous laser ablation is usually a last-resort for cosmetic vein treatment since the majority of EVLA procedures are covered by insurance and performed for medical reasons. Our vein specialists have performed tens of thousands of endovenous laser ablation procedures with successful treatment results.


This technique is usually reserved for removal of twisted or lumpy veins that bulge or protrude from the skin.
Ambulatory phlebectomy (AP) removes bulging surface veins during a relatively quick in-office procedure. Using local anesthesia, tiny surgical incisions are made on the skin above the targeted veins. A special thin hook is then used to pull out the vein. Following an ambulatory phlebectomy procedure, patients are instructed to wear a compression bandage and compression stockings for several days. Hot tubs, long-distance traveling, and heavy lifting are advised to be avoided for at least a week. Normal activities may otherwise be resumed after treatment. This procedure may be performed for cosmetic or medical reasons, depending on the patient’s vein condition and vein treatment coverage of their insurance company. Visible scars on the skin surface may result from placement of tiny surgical incisions placed to facilitate hook removal of the vein (phlebectomy).

Results of Cosmetic Vein Treatments

The amount of time it takes to obtain visibly noticeable results following a cosmetic vein treatment varies according to the type of treatment and the extent of the vein condition being treated. Smaller veins usually shrink, collapse, and disappear more quickly than larger ones. Vein treatments that surgically remove the targeted surface veins such as ambulatory phlebectomy can result in immediate disappearance of the once visible vein, but treatment area, bruising, swelling, discoloration, and skin incisions may often take weeks to months to resolve and heal completely.
