How Does Hair Grow?
Whether we are born with a smooth, shiny head, or hair like Elvis, we are born with about 100,000 hair follicles. As we age, we naturally shed about 60 hairs a day. While it is a common misconception that hair shedding is the cause of hair loss, the hair-shedding follicles still remain active, meaning another hair has the potential to regrow.
Hair growth is an ongoing three-phase process including anagen, catagen, and telogen phases. The anagen phase is the growing phase of hair, lasting anywhere between 2-8 years. The catagen phase is the phase in which some unknown signal causes the hair bulb at the base of the follicle to detach from the scalp. The telogen phase is the final stage of the hair cycle. Once this final stage occurs, the regrowth process then starts over.
Why Do We Lose Hair?
Over time, hair follicles can become less active or they can become inactive altogether, leading to visible thinning and baldness. Men and women are both prone to what is commonly referred to as genetic pattern baldnes of which roughly 70% of men and 40% of women are affected. The level or degree of response to a single biocellular hair restoration treatment cannot be predicted or known in advance. Painless Ring Block Anesthesia (RBA)
A Coban elastic tourniquet is placed circumferentially around the scalp. A ring block with injected local anesthetic agent (lidocaine/epinephrine/bicarbonate) provides complete anesthesia to allow painless injection of biocellular agents and scalp microneedling to be performed. The entire biocellular hair restoration treatment takes about 90-180 minutes to perform, depending on which biocellular treatments are being done. The treatment is done in a private, office-based setting. No special wound care is needed.
Following a biocellular hair restoration treatment at Hogue Clinics, your scalp will be rinsed in our procedure room sink and you will go home looking essentially the same as you did prior to the procedure. There will be minimal to no bleeding or downtime due to the rapid healing effects of the injected HD-PRP. It is of vital importance to avoid taking any aspirin, NSAIDs, or blood thinners for 10 days prior to a biocellular hair restoration treatment. Mild tissue swelling from the ring block anesthesia infrequently affects the forehead and upper face area, along with the potential for bruising. A Velcro cloth headband is recommended to be worn for 24-48 hours to minimize facial swelling and edema.

Prior to the integration of micronized fat into our biocellular hair restoration treatment offerings, HD-PRP plus Cell® (extracellular matrix) with scalp microneedling had been our most popular hair restoration treatment at Hogue Clinics. Extracellular matrix (ECM) exists in the biological makeup of all species. This product is loaded with signals that instruct cells to do specific things such as heal the skin and stimulate hair follicle rejuvenation. ECM attracts adult stem cells and promotes an increased blood supply to the area. When used for treating hair loss, hair thinning, hair miniaturization, and early balding, ECM appears to restore the necessary cellular conditions responsible for healthy hair growth. The Treatment Process
The process of collecting HD-PRP involves obtaining whole blood from a patient’s vein. The recommended amount of whole blood depends on the amount or extent of scalp area that needs to be treated.
The option of either a single volume of 180-mL, or a double volume of 360-mL of whole blood, is drawn from a patient’s arm vein. If hair loss is restricted to the top of the head, crown, frontal region, and temple areas, then a single volume of blood is usually sufficient. If the entire scalp exhibits hair loss or thinning, then a double volume of blood may be recommended. The whole blood is placed in our Angel® Whole Blood Separation System, spun down to isolate high-density platelet-rich plasma (HD-PRP). From the original 180-mL to 360-mL of blood, approximately 20-mL to 40-mL of HD-PRP is obtained for use during the biocellular hair restoration treatment. This 180-mL or 360-mL whole blood amount drawn for a single Mega HD-PRP hair restoration treatment at Hogue Clinics is significantly more blood than what most other hair restoration specialists harvest for a treatment session.
This is why we routinely only perform one biocellular hair restoration treatment, then wait six months to evaluate the treatment response, whereas other hair restoration specialists often recommend multiple treatments performed at weekly to monthly intervals. The collection of whole blood to isolate HD-PRP is done since platelet-rich plasma contains a highly enriched number of platelet cells that promote the body’s natural healing process by releasing powerful growth factors, cytokines, proteins, and peptides to collectively stimulate hair, skin, and tissue regeneration. HD-PRP alone can be used for biocellular hair restoration, or it can be more effectively be combined with micronized fat and ACell®.
The process of collecting micronized fat involves harvesting fat from a patient’s body, usually from the abdomen or flank area using tumescent local anesthesia. The collected fat is washed then micronized into very tiny fat tissue droplets which can be easily injected through tiny 27 gauge needles into the scalp. Micronized fat tissue contains progenitor cells, stem cells, stromal cells, and lipocytes that promote the body’s natural healing process to collectively stimulate hair regeneration. The use of micronized fat is an integral part of the ALMI Hair procedure. For the ultimate in biocellular hair restoration, micronized fat, HD-PRP, ACell®, and scalp microneedling are all used during a single treatment session.
Recovery and Results
A huge attraction to biocellular hair restoration is that there is no downtime. Swelling and edema may initially occur after treatment, but will usually subside quickly within a week. For best recovery, it is advised that patients avoid any strenuous activity for the first couple of days and avoid face sleeping. The use of biocellular agents such as HD-PRP will help speed the recovery time. Because this treatment is a non-surgical form of hair restoration, it causes little to no scarring.
Depending on an individual’s regenerative capacity, treatment results and downtime will vary from person to person. Hair regeneration results following a biocellular hair restoration treatment are evaluated at 6 months, although maximal results may take up to two years. If a good response occurs after the initial treatment, then additional treatments may be considered to achieve the best additive results.
