Hair loss is an unpleasant and embarrassing condition that both men and women experience over the course of their lives, some earlier than others. Wearing a hat or sporting certain hairstyles to cover thinning hair or a receding hairline only masks the condition. When people finally get around to addressing their hair loss situation, they tend to first consider surgical hair transplantation, since that is a familiar phrase. Hogue Clinics wants men and women to break from the ‘familiar’ and consider the completely natural remedy, biocellular hair restoration.
The use of autologous high-density platelet-rich plasma (HD-PRP) and fat aspirate concentrate (FAC) is a breakthrough biocellular hair restoration approach to hair loss. Using the ALMI Hair procedure, regenerative medicine specialists at Hogue Clinics use your own body’s fat cells to improve the function of scalp hair follicles.
The ALMI Hair procedure uses patient-derived HD-PRP and fat aspirate concentrate (FAC) to enhance hair growth, fight the progression of hair loss, and support hair follicle function. The ALMI Hair procedure is unlike any other hair restoration procedure, thereby making the familiar hair transplantation a consideration of last resort for early hair loss or mild genetic pattern baldness, especially when hair thinning or hair miniaturization is a predominant concern.
Effective biocellular hair restoration is provided at Hogue Clinics to to treat hair loss & thinning for male and female genetic pattern baldness. While hair loss treatment is not a one-size-fits-all approach, a customized treatment plan by regenerative medicine specialists at Hogue Clinics use information derived from physical examination, hair loss history, the degree of hair loss, the rate of progression, and response to prior treatments.

How Does Hair Grow?
Whether we are born with a smooth, shiny head, or hair like Elvis, we are born with about 100,000 hair follicles. As we age, we naturally shed about 60 hairs a day. While it is a common misconception that hair shedding is the cause of hair loss, the hair-shedding follicles still remain active, meaning another hair has the potential to regrow.
Hair growth is an ongoing three-phase process including anagen, catagen, and telogen phases. The anagen phase is the growing phase of hair, lasting anywhere between 2-8 years. The catagen phase is the phase in which some unknown signal causes the hair bulb at the base of the follicle to detach from the scalp. The telogen phase is the final stage of the hair cycle. Once this final stage occurs, the regrowth process then starts over.
Why Do We Lose Hair?
Over time, hair follicles can become less active or they can become inactive altogether, leading to visible thinning and baldness. Men and women are both prone to what is commonly referred to as genetic pattern baldnes of which roughly 70% of men and 40% of women are affected. While there are a multitude of reasons overall for why hair loss occurs, some of the more common reasons for hair loss include:
- Androgenetic alopecia
- Certain health conditions, such as hypothyroidism
- Hormone imbalances
- Tight, pulled-back hairstyles
About HD-PRP, ACell®, Fat Aspirate Concentrate (FAC), and Scalp Microneedling
Biocellular hair restoration, a non-surgical alternative to hair transplantation, is offered at Hogue Clinics for women and men suffering from hair loss, hair thinning, hair miniaturization, and early balding. Treatment is performed on the scalp using ring block anesthesia to allow for a completely painless experience during biocellular injections and scalp microneedling.
Dr. Hogue, a cosmetic surgeon and regenerative medicine specialist, uses a combination of high-density platelet-rich plasma (HD-PRP), ACell® (FDA-cleared porcine-derived extracellular matrix used Off-Label for hair restoration), fat aspirate concentrate (FAC), and scalp microneedling for biocellular hair restoration.
The level or degree of response to a single biocellular hair restoration treatment cannot be predicted or known in advance since each individual has unique regenerative capabilities. Results vary depending on hair loss causation and the individual’s regenerative capacity. At Hogue Clinics, a single hair restoration treatment is performed, then respone to treatment is evaluated at six months. While visible results may be seen as early as several weeks, follow up photos are obtained at six months following a biocellular hair restoration treatment.

Benefits of Biocellular Hair Restoration at Hogue Clinics
Since high-density platelet-rich plasma (HD-PRP) and fat aspirate concentrate (FAC) is harvested from your own blood and fat tissue, respectively, there is no risk of tissue rejection. ACell® is a sterile, non-cellular, FDA-cleared biologic agent. The combination of HD-PRP, FAC, and ACell® is a way to naturally influence the cycling of hair follicles, to regenerate scalp vasculature, to improve the function of hair follicles, and to reverse the process of hair loss and hair thinning.
Painless Ring Block Anesthesia (RBA)
A Coban elastic tourniquet is placed circumferentially around the scalp. A ring block with injected local anesthetic agent (lidocaine/epinephrine/bicarbonate) provides complete anesthesia to allow painless injection of biocellular agents and scalp microneedling to be performed. The entire biocellular hair restoration treatment takes about 90-180 minutes to perform, depending on which biocellular treatments are being done. The treatment is done in a private, office-based setting. No special wound care is needed.
Following a biocellular hair restoration treatment at Hogue Clinics, your scalp will be rinsed in our procedure room sink and you will go home looking essentially the same as you did prior to the procedure. There will be minimal to no bleeding or downtime due to the rapid healing effects of the injected HD-PRP. It is of vital importance to avoid taking any aspirin, NSAIDs, or blood thinners for 10 days prior to a biocellular hair restoration treatment. Mild tissue swelling from the ring block anesthesia infrequently affects the forehead and upper face area, along with the potential for bruising. A Velcro cloth headband is recommended to be worn for 24-48 hours to minimize facial swelling and edema.
Good candidates for a Biocellular Hair Restoration treatment includes the following:
- Men and women who prefer a less invasive, more natural, non-surgical hair restoration treatment method to prevent ongoing and future hair loss, as well as to promote hair growth and thickening of miniaturized hairs and thinning hairs.
- Men and women in the early stages of female or male genetic pattern baldness, or anytime there is an abundance of miniaturized or thinning hairs on the scalp.
- Patients should be in overall good health and have no underlying blood conditions or autoimmune diseases that mightly negatively influence the treatment results.
- Patients should be non-smokers and avoid the use of aspirin, NSAIDS, and blood thinners for at least 10 days prior to a biocellular hair restoration treatment.
Biocellular Hair Restoration Treatment Options at Hogue Clinics
HD-PRP with Scalp Microneedling using Ring Block Anesthesia
The most simple, minimal biocellular hair restoration treatment at Hogue Clinics involves the use of high-density PRP (HD-PRP) with scalp microneedling. Ring block anesthesia is used for a completely painless injection experience. This treatment takes about 45 minutes to perform.
HD-PRP plus ACell® with Scalp Microneedling using Ring Block Anesthesia
Prior to the integration of fat aspirate concentrate (FAC) into our biocellular hair restoration treatment offerings, HD-PRP plus Cell® (extracellular matrix) with scalp microneedling had been our most popular hair restoration treatment at Hogue Clinics. Extracellular matrix (ECM) exists in the biological makeup of all species. This product is loaded with signals that instruct cells to do specific things such as heal the skin and stimulate hair follicle rejuvenation. ECM attracts adult stem cells and promotes an increased blood supply to the area. When used for treating hair loss, hair thinning, hair miniaturization, and early balding, ECM appears to restore the necessary cellular conditions responsible for healthy hair growth. Ring block anesthesia is used for a completely painless injection experience. This treatment takes about 1 hour to perform.
ALMI Hair (HD-PRP + FAC) with Scalp Microneedling using Ring Block Anesthesia
ALMI Hair (autologous lipocyte micronized injection for hair loss) is a breakthrough regenerative hair treatment now offered at Hogue Clinics. The ALMI Hair treatment uses HD-PRP combined with FAC that is injected into the scalp. Microneedling of the scalp facilitates the mixing of the fat cells and biocellular agents, as well as provides mechanical injury to the scalp tissue to promote tissue regeneration. Ring block anesthesia is used for a completely painless injection experience. This treatment takes about 2.5 hours to perform.
ALMI Hair (HD-PRP + FAC) plus ACell® with Scalp Microneedling using Ring Block Anesthesia
ALMI Hair (Autologous Lipocyte Micronized Injection for Hair regrowth) is a breakthrough regenerative hair treatment now offered at Hogue Clinics that may be combined with ACell® for the ultimate in biocellular hair restoration. This treatment uses HD-PRP combined with FAC plus ACell®, each individually injected into the scalp. Microneedling of the scalp facilitates the mixing of the fat cells, biocellular agents, and bioactivation of injected platelets within the scalp, as well as provides mechanical injury to promote tissue regeneration. Ring block anesthesia is used for a completely painless injection experience. This treatment takes about 2.75 hours to perform.
The Treatment Process
The process of collecting HD-PRP involves obtaining whole blood from a patient’s vein. The recommended amount of whole blood depends on the amount or extent of scalp area that needs to be treated.
The option of either a single volume of 180-mL, or a double volume of 360-mL of whole blood, is drawn from a patient’s arm vein. If hair loss is restricted to the top of the head, crown, frontal region, and temple areas, then a single volume of blood is usually sufficient. If the entire scalp exhibits hair loss or thinning, then a double volume of blood may be recommended. The whole blood is placed in our Angel® Whole Blood Separation System, spun down to isolate high-density platelet-rich plasma (HD-PRP). From the original 180-mL to 360-mL of blood, approximately 20-mL to 40-mL of HD-PRP is obtained for use during the biocellular hair restoration treatment. This 180-mL or 360-mL whole blood amount drawn for a single Mega HD-PRP hair restoration treatment at Hogue Clinics is significantly more blood than what most other hair restoration specialists harvest for a treatment session.
This is why we routinely only perform one biocellular hair restoration treatment, then wait six months to evaluate the treatment response, whereas other hair restoration specialists often recommend multiple treatments performed at weekly to monthly intervals. The collection of whole blood to isolate HD-PRP is done since platelet-rich plasma contains a highly enriched number of platelet cells that promote the body’s natural healing process by releasing 30+ powerful growth factors, cytokines, proteins, and peptides to collectively stimulate hair, skin, and tissue regeneration. HD-PRP alone can be used for biocellular hair restoration, or it can be more effectively be combined with micronized fat and ACell®.
The process of collecting fat aspirate concentrate (FAC) involves harvesting fat from a patient’s body, usually from the abdomen or flank area using tumescent local anesthesia. The collected fat aspirate is centrifuged and filtered to isolate fat aspirate concentrate (FAC), which can be easily injected through tiny 25-Gauge or smaller needles into the scalp. For the ultimate in biocellular hair restoration, FAC, HD-PRP, ACell®, and scalp microneedling are all used during a single treatment session.
Recovery and Results
A huge attraction to biocellular hair restoration is that there is no downtime. Swelling and edema may initially occur after treatment, but will usually subside quickly within a week. For best recovery, it is advised that patients avoid any strenuous activity for the first couple of days and avoid face sleeping. The use of biocellular agents such as HD-PRP will help speed the recovery time. Because this treatment is a non-surgical form of hair restoration, it causes little to no scarring.
Depending on an individual’s regenerative capacity, treatment results and downtime will vary from person to person. Hair regeneration results following a biocellular hair restoration treatment are evaluated at 6 months, although maximal results may take up to two years. If a good response occurs after the initial treatment, then additional treatments may be considered to achieve the best additive results. If there is no progress or just minimal improvement after the initial treatment, our regenerative medicine specialists may not recommend additional biocellular hair restoration treatments in the patient’s best interest.
